www.e-healthinsuranceservices.com Lifeline membership guide (from 1.4.2003) What is covered? These Notes are very important. Please read them carefully along with the table of benefits as they set out the details of your cover and apply to each person covered under your membership. What is covered - your benefits in detail All the benefit limits in this table of benefits and Notes are set out in £sterling, US$ and €Euros. The currency in which you pay us your subscriptions, being either £sterling, US$ and €Euros, is the currency that applies to your membership for the purpose of the benefit limits. For example, if you pay us subscriptions in £sterling then the benefit limits given in £sterling apply to your membership and US$ and €Euro limits do not apply to you. If you are unsure which level of cover you have, the currency that applies to your membership, or whether you have an annual deductible, please phone us. Note 3: Other benefits Depending on your Lifeline level of cover we pay for the following benefits up to the amounts shown below and in the table of benefits. Note 3c: for Gold level only. Emergency dental treatment. If you are covered under the Gold level we pay up to a total amount of £sterling400, US$700 or €Euro600 (April 2003 rates) as applicable each membership year for emergency dental treatment that you receive from your dental practitioner during your first visit and the 14 days immediately following that first visit for each dental emergency you have during the membership year. We do not have to pay for any dental treatment, related to that dental emergency, that you receive after the 14 days. By eligible emergency dental treatment we mean emergency dental treatment consisting of: * dental examination * radiography ( for example an X-ray) * replacement of a lost filling * extraction of a tooth (or tooth root) * stopping abnormal heavy bleeding (haemorrhage) * cutting into an abscess * dressing a root canal * prescribed antibiotics * re-cementing a crown, bridge or inlay * adjustment or repair of a denture * construction and fitting of a temporary crown * call-out charge
